Submit a Summary of Your Project
Please use the Grant Reporting Form listed below to answer these questions following the completion of your Grant Project. This report must be submitted by November 1 of the year your grant was awarded. Failure to submit this report in a timely manner will disqualify you from applying for any future grants until the delinquent report is submitted.
- Give a brief description of the project, including goals and objectives.
- What underlying problem did the project address? What impact did the grant have on the problem?
- What were your organization’s achievements as they relate to the goals and objectives of the project? Any unexpected setbacks or outcomes? (Attach newspaper articles, etc. if available. Digital photos are required and should be emailed to address below.)
- Per instructions on bottom of Grant Report Form, return via mail or email by November 1.
